The LAR project has been funded by the Commonwealth
Government under the Public Health Chronic Disease and
Palliative Care Program.

400 days – 13 months
Prognostication – I wouldn’t be surprised if this person were
to die in the next 12 months.
What does this look like then for an individual?

• Person centred
• Timely
• In the place of their choosing
• Respectful of their values, hopes and beliefs
• Makes a difference to both their living and their dying
• Respects the experiences of their family and loved ones

Responding to the mental health and psychological
impacts of living with an advanced chronic and terminal
illness is a human rights issue.
• All people deserve to die with dignity,
• All people deserve exceptional palliative care
• It should become normal standard practice – not just
happen by chance
