GENERAL INFORMATION
This Post
This post provides an exciting opportunity to work within the network of Imperial
College Hospitals at St. Mary’s Hospital in the departments of Emergency Medicine
and Critical Care. This 12 month post will provide two successful candidates with 6
months experience each of Emergency Medicine and critical care. Working in these
two busy and dynamic departments will provide an excellent opportunity for those
wishing to gain front line and critical care experience in a supportive environment.
THE NEW ACADEMIC HEALTH SCIENCE CENTRE
Following Secretary of State for Health approval, the UK’s first Academic Health
Science Centre (AHSC), was formed on 1 October 2007, when St Mary’s NHS Trust
merged with Hammersmith Hospitals NHS Trust and integrated with Imperial College
London.
The new ‘Imperial College Healthcare NHS Trust’ is the foundation for the AHSC,
and the largest NHS Trust in the UK.
St Mary’s Hospital
St Mary’s NHS Trust was formed on 1st April 1993 and comprises St Mary’s Hospital,
the Western Eye Hospital, and acute services at St Charles Hospital. St Mary’s NHS
Trust is a constituent member of Imperial College School of Medicine (ICSM).
The Trust is responsible for an area of approximately 13 square miles and comprises
part of the City of Westminster and Royal Borough of Kensington and Chelsea. The

Trust provides a comprehensive range of acute and specialist services to a local
community of approximately 337,900 residents. In addition the Trust provides a
number of specialist services to patients outside the local area. The Trust has 541
beds and employs over 2,500 people.
St Mary’s is situated in an uniquely accessible position, being both adjacent to the
West End, Paddington Station and to the Heathrow Link.
The Hospital became one of four Major Trauma Centres for London in December
2010.
St Charles Hospital, London W10
St Charles’ Hospital is managed by the local Primary Care Trust. St Mary’s runs a
neuro-rehabilitation ward and orthopedics and rheumatology out patient services.
There is a also a Minor Injuries Unit run by Nurse Practitioners.
Western Eye Hospital, Marylebone Road
The Western Eye Hospital is internationally renowned for it’s highly specialised
services which includes special care to patients with eyelid problems or artificial
eyes.

Hammersmith Hospitals NHS Trust (HHNT) has a worldwide reputation as a centre
for excellence in patient care, medical science and teaching. It comprises three
outstanding hospitals in west London:

The Engineering, Science and Management departments are based at the main site
in South Kensington. There are approximately 10,000 students and some 5,000 staff,
including approximately 2,500 academic and research staff. The number of
postgraduate students in relation to total student numbers is higher than for any other
UK university and reflects the College’s world-wide reputation for research
excellence.
The Imperial College School of Medicine was established in its present form on 1 st
August 1997 by the merger of the Royal Postgraduate Medical School and Charing
Cross & Westminster Medical School, the National Heart and Lung Institute (Royal
Brompton Hospital) and St Mary's Hospital which had joined Imperial College in
1988.
The expanded ICSM is one of the largest medical schools in the UK with a significant
research activity. Basic medical sciences will be taught at the South Kensington
Campus and academic clinical groups will be located at, or adjacent to, St Mary’s,
Charing Cross, Chelsea & Westminster, Royal Brompton and Hammersmith
Hospitals.
Trust Grade (CT1/2) in Emergency Medicine
The Emergency department at St. Mary’s Hospital
The ED at St. Mary’s treats approximately 78,000 adult and 23,000 paediatric new
attendances per annum. In addition the department has treated in excess of 600
patients who fulfil the Major Trauma criteria.
Patients are managed by the ED multidisciplinary team which includes medical and
nursing staff, a team of Emergency Nurse Practitioners, Occupational Therapists,
Mental Health liaison team and an Alcohol Health Worker. There are close links with
our colleagues in the adjacent 42 bedded admissions ward, the Douglas Admission
and Assessment Unit.
A 12 bedded Clinical decision unit adjacent to the ED allows patients who are
awaiting time critical investigations or who require an extended period of observation
to be cared for in an appropriate environment.
The current department was opened in September 1987, with a further
redevelopment in 1999 for the adult area. The paediatric department opened in June
2000.
The department has a four bay fully equipped resuscitation room and a small
operating theatre for minor procedures as well as its own Xray facilities (see list of
facilities). There is a dedicated trauma lift to the Acute Imaging Centre on the ground
floor. Web PACS is operational throughout the Trust. The department is fully
‘paperless’ and computerised with the First Net Cerner system.There is a blood gas
facility and phlebotomy service within the department .
The paediatric department is a shared development with paediatricians and ED
doctors working alongside each other. There are currently 8 Paediatric middle grade

posts and a full time Paediatric ED specialist registrar on a rotation with Chelsea and
Westminster and the Royal London Hospitals. There is a general ED F2 and middle
grade shift allocated to paediatric A&E each day.
The paediatric and general departments share a common reception, but separate
triage and waiting areas.

Registrars hold NTN numbers and are on rotations within the North Thames (West)
region.
F2/CT1/2 Team
There is a team of 18 full time F2/CT1/2 doctors who are managed as distinctive 6
monthly teams. One of the successful applicants will join Team 60 from August 2016.
Urgent Care Facility
There is a co-located dedicated Urgent Care Centre where primary care patients and
those with minor injuries/illness are assessed by a team comprised of a GP ENP and
ED medical staff. A GP is also responsible for ‘streaming’ of patients at triage

Duties of Emergency Medicine Post
The successful candidates will work within a team of 18 junior doctors on a fixed 18
week rolling Band 1A rota (pending decision re introduction of the new contract) The
team are responsible for the initial treatment of all adult and paediatric patients
presenting the Emergency Department, with support and supervision from the
middle-grade and consultant team.
Active participation in the Education Programme, including audit, is expected and
encouraged.
Educational Content of Emergency Medicine Post
The department has always emphasised the need for education, and realises that
this attracts high calibre staff. The junior doctors’ educational programme underpins
cohesive working, and ensures that as many of the team as possible meet in an
educational setting on Tuesdays. Audit projects are presented in these sessions.
There are monthly trauma rounds and clinical governance presentations as well as
didactic sessions. There are monthly registrar educational days in addition to the
regional training days. There is protected teaching time for the Paediatric ED staff on
Thursday mornings. All educational programmed activities take place within the
Seminar room, located in the office area above the clinical space.
‘Board rounds’ in the majors area at 0800, 1200 and 1600 allows senior review of all
patients in the department, and provides an excellent focus for teaching and sharing
ideas on the management of particular patients.
All staff are encouraged to participate in the departmental audit programme. There is
a multidisciplinary audit meeting each month during the teaching programme and
there are ongoing audits in Thrombolysis, Trauma and Resuscitation.
A multidisciplinary and multispecialty Trauma Review Meeting is held weekly in
addition to Medical Grand Rounds.

DUTIES OF THE POST:
The Trust grade (CT1-CT2) will form part of a team, which usually includes two
registrars and a consultant in Intensive Care for the day to day management of
patients in a multi-disciplinary 13-bedded Intensive Care Unit. Clinical experience
will be gained in inotropic support, invasive and non-invasive ventilation,
haemofiltration, parenteral and enteral nutrition, and intra-aortic balloon
counterpulsation. Technical skills will be acquired in central venous and arterial
cannulation and the insertion of pulmonary artery catheters.
The SHO will be encouraged to attend the Basic Assessment and Support in
Intensive Care (BASIC) course organised by the Department and attend weekly
tutorials in intensive care medicine. No prior experience in Intensive Care is
expected, and applications are welcomed from trainees from all base specialties.

Pay
The post is covered by Terms and Conditions of service of Hospital and Medical and
Dental Staff (England and Wales) and the General Whitley Council Conditions of
Service. They are amended from time to time.
Pensions
This post is subject to the provisions of the NHS Pension Scheme, which is
contributory at the rate of 6% of salary. Membership of the scheme is automatic
unless election is made at anytime in favour of provision under a Personal Pension
Plan. After opting out of the scheme the employee will assume full responsibility for
pension provision and compliance with the Social Security Act 1986.
Registration
The person appointed will be required to be registered with the GMC. It is the
responsibility of staff to ensure thy keep their registration with their professional
bodies up to date. This is a statutory requirement and proof of renewal must be
produced on request by a Trust representative and failure to do so within a specified
period may lead to dismissal..

Health and Safety
The post holder must co-operate with Management in discharging its responsibilities

Infection control
The post holder must comply with all relevant policies, procedures and training
on infection prevention and control.
It is the responsibility of all staff, whether clinical or non-clinical, to familiarise themselves
with and adhere to current policy in relation to the prevention of the spread of infection and
the wearing of uniforms.
Clinical staff – on entering and leaving clinical areas and between contacts with patients all
staff should ensure that they apply alcohol gel to their hands and also wash their hands
frequently with soap and water. In addition, staff should ensure the appropriate use of
personal protective clothing and the appropriate administration of antibiotic therapy. Staff are
required to communicate any infection risks to the infection control team and, upon receipt of
their advice, report hospital-acquired infections in line with the Trust’s Incident Reporting
Policy.
Staff have a responsibility to encourage adherence with policy amongst colleagues, visitors
and patients and should challenge those who do not comply. You are also required to keep
up to date with the latest infection control guidance via the documents library section on the
intranet.
