STANDARD 3 Individual Outcomes This policy formally recognises that each person with a disability should receive a service and program that is designed to meet in the least restrictive way, his or her individual needs and goals. DVJS will ensure that service delivery will be person-centred where Participants lead and direct their service and supports: The Participant has the option of support from an independent advocate or family member if this is desired at any stage of their program. Participants are encouraged to make their own decisions and choices throughout their DVJS program. DVJS realises that it is the Participant’s life and future and respects that they have the right to live the lives they wish to lead. DVJS will try to help Participant’s achieve their goals by developing a Job Plan with them that’s designed to meet their needs and goals. Each Participant will be assigned an Employment Executive (EE) once they have been accepted onto program. The EE will work with the Participant and together, explore their needs, goals and employment requirements. Each Participant will have a Job Plan. The Job Plan is a plan that is made with full input from the Participant detailing specific objectives that enable them to reach their employment goals. Participants are encouraged to consider a range of employment options in case that vacancies in their selected industry are not available. The Participant will know exactly who will be responsible, what will be done and when it will be done so that their own progress and performance can easily be measured. Once the Job Plan is agreed to, printed and signed by the Participant, it will form the basis of their program. The current signed Job Plan will be kept in the Participant’s file and previous Job Plans destroyed. A copy of the Job Plan is offered to the Participant. The Job Plan is a compulsory component of Service Delivery & failure to sign the Job Plan will be viewed as a breach of compliance and will be reported upon to relevant Government authorities. DVJS Standard 2/ Individual Outcomes last updated Feb 2017 1 Job Plans cover mainly those needs and goals which fall within the main activities of DVJS. Assistance and liaison with other services may be necessary and may be incorporated into the Job Plan. The approach undertaken in developing the Job Plan requires full consultation with the Participant in the development, implementation and review of their Job Plan. Review & Evaluation of Job Plans Job Plan’s are flexible to cater for the changing needs and goals of the Participant and are constantly reviewed at a within every 3 month period for Participants on Employment Assistance (EA) or Post Placement Support (PPS) phases and 6 months for those in Ongoing Support (OGS) or when there is a change in circumstance or activity. Reviews are conducted on an ongoing basis until the Participant’s program is no longer active…they have withdrawn or Independent status. During a review and evaluation of the Participant’s Job Plan, the Participant and their EE consider the following… 1. Whether the agreed support has been provided. 2. What progress has been made towards achieving goals and objectives. 3. What amendments need to be made to reflect progress or changes and incorporate future goals. 4. Whether changes made are to everyone's satisfaction. Via case management, Participant’s continually participate and contribute in the evaluation and review of their Job Plan. Once the Job Plan has been reviewed and is agreed upon by both parties, the new plan will be implemented immediately. Service Delivery Staff and Management of DVJS, will make every effort to be sensitive to and meet specific needs of Participant’s in reference to their: Age, Gender, Nationality, Religious Affiliation, and Ethical beliefs. DVJS endeavours to meet the specific needs of its Participant’s. Participant’s may request to work with particular staff members to meet their needs. Every effort will be made to maintain an even equilibrium of staff male/ female ratios. DVJS will make every reasonable effort to assist the Participant obtain work that meets their skills and interests. At times, DVJS may present the Participant positions that may not be on their Job Plan if the position is deemed to be suitable. The Participant is not obligated to accept any offer of employment but must give legitimate reason for not accepting employment that is on their Job Plan or suitable. DVJS Standard 2/ Individual Outcomes last updated Feb 2017 2 DVJS will not assist Participant’s obtain employment that may place either them at risk or DVJS in a precarious position. Some examples of positions that would not be sought on a Participant’s behalf are… Topless Bar staff, Brothel Receptionist, Sex Talk etc. Non Disclosure DVJS respects that Participants may wish to find their own employment and not disclose to their employer that they have a disability and/or connected to DVJS. In these circumstances, service delivery will be restricted to Participants when they are at work as DVJS has not arranged employment and therefore cannot provide onsite support & financial incentives. DVJS will only provide off-site support to the Participant and look for alternative employment should the job become ‘in-jeopardy’. DVJS will not contact the employer about the Participant. In the event that the Participant would like disclose to their employer about their disability, DVJS will provide information to assist them make an informed decision about consequences of disclosure. DVJS will not make contact with the employer on their behalf. In the event that the Participant would like DVJS to contact their employer, DVJS will not make contact. Once disclosure has been made there is no return and DVJS respects that people do ‘change their minds’. DVJS Standard 2/ Individual Outcomes last updated Feb 2017 3
