SRVA Registration Process for A5 INDOOR and BEACH Players
Here are the instructions to register for the SRVA and USA Volleyball. DO NOT SEND payment
to the SRVA office if you are playing for A5. A5 will submit the required paperwork along with
membership payment.
Go to srva.org – and click on “Member Registration”.
If you are a returning member, select the green button “Click for RETURNING Members”; If you
are new and have never been a member of the SRVA, select the yellow button “Click to
Register as a NEW Member”.
Note: throughout the sign-up process, red pop-up boxes will show up – just click “ok”

Enter “Last Name” and “Birth Date” of player, then click “Find Person” if you have previously
played in the SRVA. If this is your first time registering/playing in the Southern Region, select
“Register New Member”.
If you have previously played for the SRVA, the Player’s Name and Email will appear in the
respective boxes. Enter the password and click on the button to left of the cell where you
entered the password (see yellow arrow below). If you have forgotten the password, click on the
“Forget Password” button and enter the requested information and then click “Next”.

Confirm information is correct, then click “Next”.
Select the applicable check box. The first/top check box is for those players trying out for
INDOOR, the 2nd/bottom check box is for those players only trying out for BEACH, then click
“Next”.
INDOOR Players: Select the check box for ($60) Junior Player

BEACH Players: Select the check box for ($25) Outdoor (Beach) or Recreational Junior Player,
then select “Next”.
Review and update (if applicable) information in this section. You will likely have to re-enter your
password and confirm/re-type (see red arrows below), then click on “Save/Update”.
Final verification screen will appear, once confirmed, click “Register Returning Member”. [Note:
the “Individual Level” will still show as 2015-16 Age Level. The correct Eligibility Age will show
once the player has been registered.]

Follow the instructions highlighted in yellow [Read and Indicate Acceptance of all Terms and
Conditions by Checking All 4 Boxes Below]. Enter the Applicants Initials in the designated box.
This will serve as your electronic signature/verification. Click on the “Submit Verification Code
(E-Sign)” button. Once you submit your registration, you will not be able to make any changes
without contacting the SRVA office.

The final step is to PRINT and SAVE. The Individual Membership Form and the Medical
Release Form.
Click on “Create Printable Individual Membership Form PDF” button (yellow) in the top left
corner. Save a copy for your records (right-click with your mouse and select “Save As”. You can
always sign back into the SRVA online registration and print/save in the future as well. Bring a
COPY of this form with you to Commitment Night.
Click on “Create Printable Medical Release with Notary Info PDF” button (yellow) in the top right
corner. Print a copy and fill out the remainder of the form. Players CANNOT participate without
this form. Player and parent/guardian must sign the form. You do not need to get the Medical
Release notarized if you play for A5. A club notary will take care of notarizing. Bring a COPY of
this form with you to Commitment Night.
Once you have printed/saved both documents, click “Exit”.
As a reminder, you SHOULD NOT SEND payment to the SRVA office if you are playing for A5.
A5 will submit the SRVA/USAV Registration Form, along with applicable membership payment.
The Medical Release form should not be sent to the SRVA. Make sure to keep a copy for your
records.
