OCCUPIED UNIT RELEASE FORM _______________________________ Community Name _________________ Unit Number _________________ Installation Date _______________________________ Community Fax _________________ Community Phone _________________ Confirmation # Dear Resident: Your unit has been selected to receive new floor covering. The installation may cause you some minor inconvenience; however, the end result will be rewarding. In order to assist us in performing this service efficiently and professionally, we ask your cooperation in completing the following instructions prior to the arrival of the installation crew. 1. 2. We will only move your large items of furniture and replace them to their original position. Please protect your breakables by moving them yourself. After all, no one will move them as carefully as you will. For example, lamps, plants, books, aquariums, glass table-tops, crystal, china, small furniture, items in closets, and items from under beds should be removed before the installation technician/crew arrives. 3. Appliances (refrigerators, washers, dryers) and large articles of furniture (china cabinets, entertainment centers, armoires, dressers, etc.) must be emptied of their contents. 4. Disconnect and remove all equipment and appliances (computers, printers, stereos, speakers, televisions, video or DVD players, washers, dryers, and refrigerators, etc.). Removal of appliances are optional, but only if emptied and agreed upon before arrival (additional fees apply). We will not disconnect or reconnect equipment. 5. Remove all bedding materials including pillows. Large beds must be disassembled prior to arrival of the installation crew. If you have a waterbed, it must be drained. We are not responsible for any water damage from waterbeds. 6. We will not move any type of piano, grandfather clock, television, or computer system(s). 7. Items of high value are your responsibility and must be removed from the apartment or put in a secure place prior to installation. Criterion Brock will not be responsible for unsubstantiated claims of missing items. 8. We will not be responsible for any accidental damage to furniture and/or fixtures incurred before, during, or after the floor covering installation. While care is taken in moving furniture, please note small nicks may occur. 9. No installation will be done with animals loose in the unit. Please keep them enclosed in an area which is not designated to receive new floor covering, or remove them from the premises. 10. You should keep in mind that freshly painted and wallpapered walls will mark easily, and although we are as careful as possible, some minor marking of baseboards and walls should be expected. We cannot be held responsible for any needed touch-up. 11. Understand that dust can be generated as a result of the removal of flooring and precautions should be taken to cover items. 12. Floor covering technicians are not professional furniture movers. Our installers give their very best effort to take care in handling all items, but on rare occasions, accidents do happen. If you have a concern about any possible damage to an item, you should move the item(s) yourself or have the item moved by a professional mover who is prepared to insure the activity. * If a scheduled installation must be postponed or cancelled because the above instructions have not been completed or precautions taken, a service charge will be assessed and the installation will be rescheduled. Resident has read and acknowledges the above guidelines and precautions. Resident understands and agrees that Criterion Brock will not be held responsible for damaged or missing items resulting from noncompliance on the part of the Resident. Please sign and fax to Criterion Brock at (510) 441-1609. Without receiving this form signed, we are unable to schedule and install your unit. ____________________________ Resident’s Signature __________________________ Community Manager’s Signature
